Providing good quality patient/client- centred care is the business of all health care workers. However, the care that you have observed/delivered in your clinical placements and the communication within teams, patients and their carers may not always have been at the level of best practice. As beginning level or advanced practice clinicians the question can be asked is this acceptable, and if the answer is no, what is it that you can do about it? This assignment is about exploring issues related to best practice and how you can play a part in improving care for patients.
Identify a problem encountered as a nurse in hospital setting. You can choose either from the examples below or any other similar problem encountered and follow the topic headings to develop a quality improvement project.
Some topic examples are:
medication errors, poor transition of care, patient falls, development of pressure injuries, poor fundamental care, staff bullying, poor teamwork.
